3) Application Fee of 1000 RMB

4) Teacher Recommendation Form (must be on official letterhead paper and sealed)

5) Medical History Form

6) Photocopy of Passport Title Page for Applicant, Father, and Mother

7) Recent Standardized Test Scores (e.g. CTP, SAT, PSAT, TOEFL, IELTS, SSAT, SLEP). If recent scores are not available, applicants are subject to a FIS pre-entry test.

8) At least two pages of recent writing sample demonstrating the applicant's ability to write independently.

9) At least two pages of recent mathematics homework, quiz, or test papers.
